SALARY RANGE: DOE
FLSA STATUS: Exempt

JOB PURPOSE: Serves as lead communications and services liaison between Tribal patients, their families, and available clinic health services. Oversees tribal caregivers to ensure appropriate services and a safe environment are provided for Tribal patients at all times

JOB DUTIES AND RESPONSIBILITIES: (Duties and responsibilities may be added, deleted, or changed at the sole discretion of Sycuan management at any time.)

Licensed Vocational Nurse:
• Supervises home health care to Tribal patients to promote wellness
• Monitors health care instruction to patients, family members, Caregivers, and Medical Assistants
• Ensures follow up to referrals from clinic health care providers and patient self-referrals
• Supervises and monitors basic nursing and patient assistance to Tribal patients including the accurate measuring and recording of vital signs and medications
• Informs Clinic Operation Manager of Caregiver and Medical Assistant training needs
• Supervises Tribal Caregivers and Medical Assistant patient care procedures such as dressing changes, foot care and inhalation therapy
• Monitors self-help instruction to Tribal patients and families to assist patients in their care
• Ensures pertinent written documentation regarding Tribal patient needs is forwarded to providers in a timely manner
• Monitors Tribal patient charts for accurate and complete information, including daily activities, medication taken, and nutritional intake
• Monitors encounter documents for complete and accurate data and supervises submission of monthly statistical reports
• Ensures adherence to standards for infection control in accordance with applicable local, state, and federal OSHA regulations
• Assists with training and orientation of Tribal Caregivers
• Monitors day-to-day Caregiver activities and quality of care provided to Tribal patients, including spot checks to review in-home charts and Caregivers providing services.
• Assesses Caregivers ability to provide a safe environment for Tribal patients
• Assesses ability of Caregivers to identify and report significant changes in Tribal patients’ conditions as well as family member concerns
• Assesses ability of Caregivers to provide meals according to nutritional and medical needs
• Assesses Caregivers documentation of patients taking medications as prescribed by provider, accurate vital signs, and daily activities
• Acts as liaison between Tribal patients, their families, Caregivers, and Clinical Operation Manager
• Coordinates resolution of issues pertaining to concerns or complaints related to Caregivers
• Performs other duties as assigned

Clinical Responsibilities:
• Performs patient intakes including, but is not limited to, taking patient’s history, preparing patients for examination by the provider, performing patient care procedures according to provider’s orders, and ensuring coordination of referrals to other providers and/or community agencies when ordered
• Maintains and/or ensures maintenance of referral log
• Provides relevant patient education and counseling
• Ensures documentation of all patient care services in the medical record
• Coordinates with other staff in the development, revision and implementation of policies and procedures necessary for Certification, Occupational Safety Health Administration guidelines, and Quality Improvement
• Ensures maintenance of medication cabinets, including prescription and non-prescription drugs
• Ensures monthly checks for expired medications and documentation of medical dispensing log
• Performs and/or ensures daily temperature checks on refrigerator and documents, and results in nurse’s daily log.
• Conducts monthly checks for expired medications and immunizations and ensures timely reordering of medications and immunizations
• Orders medical supplies for the Clinic and provides a supply list for all new items to the Clinical Operation Manager for approval
• Checks expiration dates monthly, as required and documents information according to established procedures
• Performs and/or ensures maintenance of immunization log and ensures preparation of quarterly immunization report for the Clinic
• Performs and/or ensures weekly check of Oxygen tanks for fullness
• Oversees maintenance of medical equipment in the Clinic, including yearly calibration and safety checks
• Performs other duties as assigned

JOB SPECIFICATIONS:
EDUCATION & EXPERIENCE
Essential:
• Current California Vocational Nurse License
• Current CPR Certification
• Minimum of five years experience in a medical office or community clinic
• Minimum of two years supervisory experience
Desirable:
• Tribal clinic experience
• Community Health Representative experience
• Phlebotomist certification

Knowledge of applicable Federal, State, and Local laws and regulations; County Programs, Medi-Cal, Medicare, CHDP, CMS, and Medi-Cal managed care programs

Desirable: Multi-lingual (English-Spanish)
