Program Coordinator

San Ysidro Health Center is recruiting a Program Coordinator/Supervisor to oversee one of its Diabetes and Cardiovascular Disease risk factor screening Program through the Center for Latino Research and Health Promotion.  SYHC’s Center for Latino Research & Health Promotion (Center) augments the health center’s clinical care with health promotion, research and care coordination programs for patients and community members. The Center’s Program Coordinators are responsible for the coordination of assigned programs within the Center. The Center’s Program Coordinators’ responsibilities include, but are not limited to: Coordinating the implementation and delivery of research and health promotion projects; coordinating a variety of duties to support the day-to-day operations of the program(s); interfacing with clinical staff and the community; report generation & submission; program evaluation; and the facilitation of program integration across SYHC sites.

The Salsa en la Comunidad program provides community outreach and education on risk factors associated with Diabetes and Cardiovascular Disease, as well as free comprehensive screening in the community consisting of risk factor surveys, height, weight and BMI calculations, blood pressure and a full lip panel via finger stick.

Program Coordinator

1.      Supervises and/or implements all program education, case management, and outreach activities, as well as the progress made toward meeting stated goals and objectives through San Ysidro Health Center’s Total Quality Management process.

2.      Coordinates, implements and monitors programs and activities to meet identified needs ensuring coordination with existing center clinical and administrative systems.

3.      Responsible for the development and implementation of program related curriculum.

4.      Establishes and maintains external linkages and provides information to the professional and lay community regarding those programs and criteria for eligibility.

5.      Interprets and applies specific procedures in organizing, monitoring, and performing the operational/research activities of the program.

6.      Coordinates on-going staff trainings in cultural competency, target health disparity related topics and other activities related to professional development.

7.      Represents the Health Center at various community meetings.

8.      Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.

9.      Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.

10. Maintains program specific records as required; submit reports, including activity, contact and mileage reports in accordance with agency policies and procedures.

11. Attends meetings and/or training sessions as required and participates on committees as directed.

12. Performs other related duties as assigned or requested.

Specific Program responsibilities:

1.      Attends community screening events.

2.      Develops community/internal event calendars.

3.      Communicates and fosters partnerships with community based organizations.

4.      Adheres to all County guidelines for screening events.

5.      Coordinates program related events and activities.

6.      Documents activities in reports and logs.

7.      Orders supplies and equipment.

The ideal candidate is bilingual (Spanish/English), organized and detail oriented, has experience supervising or leading a team, and some experience working with diabetes or CVD programs. Interested candidates should send their resume and cover letter to Ilana Brongiel at igoldberg@syhc.org .
