COASTAL
Screen patients for Medi-Cal/Healthy Families eligibility and assist in filling out Medi-Cal/Healthy Families applications. Conduct community outreach activities to identify eligible uninsured children, ages 0-5 as well as uninsured pregnant women. Increase program retention rates by providing timely follow-up to targeted families. High school diploma. Bilingual (Spanish/English) required. Certified Healthy Families Application Assistant or eligible for certification. Knowledge of local health care environment. Experience focus groups, leading presentations and needs assessments, and community mobilization preferred. Experience in community setting.
