Nurse Practitioner

SUMMARY

Provides health care services to patients under the direction and supervision of a Medical Doctor.  Perform clinical assessments of care management services in accordance with standardized procedures.

ESSENTIAL DUTIES/RESPONSIBILITIES

Comply with established departmental and administrative policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards and all other policies as required.

Examines patient, performs comprehensive physical examination, and complies patient medical data, including health history and results of physical examination.

Administers or orders diagnostic tests such as x-ray, electrocardiogram, and laboratory tests, and interprets test result for deviation from normal.

Prescribes or recommends drugs or other forms of treatment such as physical therapy, or related therapeutic procedures.

Performs therapeutic procedures such as injections, immunizations, suturing, wound care, and managing infections.

Develops and implements patient management plans, records progress notes and assists in provision of continuity of care.

Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living and health maintenance.

Discusses case with physician and other health professionals to prepare comprehensive patients care plan.

Chart review, Quality Assurance review and development, execution and periodic review of policies, procedures and medical protocol.

Submits health care plan and goals of individual patients for periodic review and evaluation by Physician.

Refers patients to Physician for consultation or to specialized health resources for treatment.

Maintain strict confidentiality of all information according to clinic guidelines.

Enhance professional growth and development through participation in adequate training’s and workshops.

Adhere to all City, State, and Federal regulation funding guidelines as related to the provision of health care and as deemed appropriate.Federal  Title 22, Federal 330 etc.

Other related duties and tasks as assigned by supervisor.

QUALITY MANAGEMENT

Display knowledge of normal signs of human development and ability to assess and provide age appropriate care.

Contribute to the success of the organization by participating in quality improvement activities.

CUSTOMER RELATIONS

Responds promptly and with caring actions to patients and employees.  Acknowledge psychosocial, spiritual and cultural beliefs and honor these beliefs.

Maintain professional working relationships with all levels of staff, clients and the public.

Be part of a team and cooperate in accomplishing department goals and objectives.

SAFETY

Maintain current knowledge of policies and procedures as they relate to safe work practices.

Follow all safety procedures and report unsafe conditions.

Use appropriate body mechanics to ensure an injury free environment.

Be familiar with location of nearest fire extinguisher and emergency exits

Follow all infection control procedures including blood-borne pathogen protocols

HIPAA/COMPLIANCE

Maintain privacy of all patient, employee and volunteer information and access such information only on a need to know basis for business purposes.

Comply with all regulations regarding corporate integrity and security obligations. Report unethical; fraudulent or unlawful behavior or activity.

MINIMUM NECESSARY

Direct Healthcare Provider – A licensed healthcare professional who provides direct or indirect patient care or consulting services.

Full Health Information Access – Access to full health information as needed for health or payment operations. Staff in this category may access and read all appropriate information.

QUALIFICATIONS:

Skills: Ability to establish and maintain professional relationships with all levels of staff, clients, and the public.  Ability to work independently, under pressure, accurately, prioritizes workload and determines priorities with minimum supervision and professionalism.  Communicate effectively and diplomatically with patients and MHCS personnel.  Willingness to be part of a team-unit and cooperate in the accomplishment of department goals and objectives.  Bilingual (English/Spanish) preferred. 

Education/Experience: High School Diploma or GED.  NP License. Licenses must be periodically renewed, and continuing education is a requirement for renewal.  Current CPR Card.  Must have the ability to be initially credentialed and then maintain credentialing by all contracted health plans.   

PHYSICAL AND MENTAL REQUIREMENTS

Normal healthcare environment.  See, hear, talk, walk and sit for long periods of time, reach, bend, stoop, lift up to 25 lbs. Repetitive hand movement; use and view PC; use calculator; telephone, operate fax, and copy machine.  Read and write; time management, interpersonal, organizational, and math skills; prioritize, and maintain confidentiality.  Have reliable transportation; clean driving record and insurance as required by the state.  MMR and Hep B series required.  Yearly Physical and TB test required.
