SUMMARY
Instructs individuals and families, in health education and disease prevention, in community health agency, by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Include the following. Other duties may be assigned.
1. Visits homes to determine patient and family needs, develops
plan to meet needs, and provides nursing services.
2. Instructs family in care and rehabilitation of patient, and in
maintenance of health and prevention of disease for family
members.
3. Gives treatments to patient following Physician's instructions.
4. Assists community members and health field personnel to assess,
plan for, and provide needed health and related services.
5. Refers patients with social and emotional problems to other
community agencies for assistance.
6. Teaches home nursing, maternal and child care, and other subjects
related to individual and community welfare.
7. Participates in programs to safeguard health of children, including
child health conferences, school health, group instruction for parents,
and immunization programs.
8. Assists in preparation of special studies and in research programs.
9. Directs treatment of patient by Licensed Practical Nurse and Home
Attendant.
10. Cooperates with families, community agencies, and medical personnel
to arrange for convalescent and rehabilitative care of sick or injured
persons.
11. Assess, observe and evaluate potential dangers of disabling conditions
and explore preventive and corrective measures.
12. Submits report to provider on patient's physical and emotional condition,
reaction to drugs or treatments. Interpret social and physical factors in
the environment that affect the clients care.
13. Performs screening tests as authorized and interprets findings to
individuals or families. Gives preventive immunization of treatments.
14. Applies nursing intervention and procedures designed to prevent and
control disease as required by local health regulations.
15. Interprets all governmental health regulations and group responsibility for
the early reporting of any sign of communicable disease, secure prompt
medical examination of individuals who are suspected of having or are
have been exposed to a communicable diseases.
16 Develops, plans and implement coordinated nursing care plans for
individuals and families. Maintains necessary records for the
establishment of priority analysis and evaluation for care. Identify
health problems by observation, interviews, analysis of records. Teach
care givers to recognize early signs of disease and deviations from
normal health, and to initiate appropriate action.
17. Study the home and community environment to identify elements
conductive to injury and accident fatigue, or emotional strain.
18. Recognize attitudes and cultural patterns that are detrimental to health.
19. Assists individuals and families to adjust and accept to families and the
community and gives guidance in the effective use of resources for
diagnosis, treatment and rehabilitation.
20. Consults with professional workers such as nutritionist and social
workers as necessary.
21. Assists patients and family with acceptance of appropriate medical
services. Interpret extent and limitations of services available.
22. Promote continuity of patient care through hospital discharge planning
and/or related health facility via referral and feedback.
23. Interpret clinic services available, and promote utilization of such
services.
24. Provides direct teaching or consultation to individuals or group with
common health or educational needs.
25. Offer guidance, support and supervision in the field to the staff
(CNA) in the implementation of the community health program.
26. Research nursing educational and professional materials, community
resources, and other agencies and programs.
27. Develops and secures culturally appropriate informational and
educational materials, guides, brochures, etc.
28. Participate in educational development activities including
departmental meetings, continue educational programs.
29. Participates in community activities, committees, workshops and
seminars.
30. Assists in the design and implementation of field investigations and
research projects.
31. Maintains patient confidentiality.
32. Performs other related duties as requested.
SUPERVISORY RESPONSIBILITIES
Manages one or more subordinate employees.
QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Bachelor's degree (B. A.) in Nursing 1-3 years recent community health care experience, must be a certified Public Health Nurse.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS
Current CPR, must be a licensed California RN with PHN certificate. Current California drivers license and safe driving record required.
